Eli Lilly launched the Zepbound Self Pay Journey Program on February 23, 2026. It discounts the 7.5mg, 10mg, 12.5mg, and 15mg doses to $449 per month, matching the 5mg rate. Without the promotion, those higher doses revert to the regular $499 to $699 range. Lilly has not published an end date for the program, but pricing pages should be re-verified quarterly. We update this page against Lilly's own pricinginfo.lilly.com on the schedule below.

    Eli Lilly launched a new single-patient-use KwikPen device containing 4 doses in one pen. Available through LillyDirect cash-pay at $299 to $449 depending on dose — the cheapest current self-pay option for higher doses.

    Vial pricing reduced

    Eli Lilly cut LillyDirect single-dose vial prices. Previous range $349-$499 lowered to current $299-$699 range depending on dose.

For most people without manufacturer-covered insurance, the cheapest path is the new LillyDirect Multi-Dose KwikPen launched in February 2026. Prices are $299 for 2.5mg, $399 for 5mg, and $449 for 7.5mg through 15mg. This is cheaper than the older single-dose vials for any dose above 5mg. If your commercial insurance covers Zepbound, the Lilly Savings Card at $25/month is still the lowest option. Income-qualified uninsured patients may also access Lilly Cares PAP for free or reduced cost.
