Zepbound Cost 2026: What You'll Actually Pay

Zepbound list price is $499 to $1,086.37 per month depending on formulation and dose. Most patients pay between $25 and $449 per month out of pocket. With commercial insurance that covers Zepbound, the Lilly Savings Card brings the cost down to $25 per month. Without insurance, LillyDirect Self Pay costs $299 to $449 per month. Medicare, Medicaid, Tricare, VA, and DOD plans are not eligible for the Savings Card, but Medicare Part D coverage begins rolling out in July 2026.

Why are there two prices?
Eli Lilly launched the Zepbound Self Pay Journey Program on February 23, 2026. It discounts the 7.5mg, 10mg, 12.5mg, and 15mg doses to $449 per month, matching the 5mg rate. Without the promotion, those higher doses revert to the regular $499 to $699 range. Lilly has not published an end date for the program, but pricing pages should be re-verified quarterly. We update this page against Lilly's own pricinginfo.lilly.com on the schedule below.

Federal framework tying U.S. GLP-1 prices to the lowest prices paid in comparable countries. Expected impact: Zepbound could drop to ~$346/month on a government discount platform launching in early-to-mid 2026. Medicare copays capped at $50/month for eligible enrollees as part of this framework.
Status: Announced, implementation in progress. Not yet reflected in point-of-sale pricing.
