Wegovy Cost 2026: Pill vs Pen Pricing Breakdown
Monthly cost by dose for both formulations, compared side by side.
Wegovy list price is about $1,349 per 28-day supply (pen). Most patients pay between $25 and $349 per month out of pocket depending on insurance status and formulation. With commercial insurance that covers Wegovy, the Savings Card brings costs down to $25 per month. Cash-pay patients can choose the Wegovy pill at $149 per month (1.5mg or 4mg starter doses through Aug 31, 2026) or the pen at $199 per month for the first 2 fills, then $349 per month. Medicare Part D already covers Wegovy for cardiovascular event risk reduction; broader weight-loss coverage begins in 2026.
What you pay, by scenario
The four most common payment paths, with the typical monthly cost and what it takes to qualify.
Savings Card applies at retail pharmacy for pill or pen. Up to 13 fills per calendar year. Maximum $100/month savings.
Most non-covered patients switch to NovoCare Pharmacy Self-Pay (pill $149, pen $349). File a formulary exception appeal first if possible.
Wegovy pill at $149 is the cheapest path. New pen patients pay $199 for first 2 fills, then $349 going forward.
Federal plans excluded from Savings Card. Medicare Part D already covers Wegovy for CV risk reduction. Obesity coverage rolls out under 2026 federal framework. 13 state Medicaid programs cover GLP-1s for obesity.
Wegovy pill 4mg at $149/mo ends August 31, 2026
Novo Nordisk's promotional pricing on the 4mg Wegovy pill expires August 31, 2026. After that date, the 4mg dose reverts to $199 per month. The 1.5mg starter dose remains at $149 ongoing. The 9mg and 25mg higher doses are $299 per month regardless. If you are considering the 4mg pill, starting before the end of August locks in the lower rate for the fills you place under the promo.
Wegovy pill self-pay by dose
Cash-pay pricing for the once-daily oral Wegovy tablet through NovoCare Pharmacy. The 4mg dose is the only promo-affected strength.
Wegovy pen self-pay by dose
