What is the difference between Wegovy pill and pen?

The Wegovy pill is an oral semaglutide tablet taken once daily, supplied in 1.5mg, 4mg, 9mg and 25mg strengths. The Wegovy pen is taken once weekly by injection. In clinical trials, the pill produced slightly less weight loss on average than the pen (13.6 percent vs 18.8 percent mean reduction at 64-72 weeks for all patients). But the pill starts lower: $149/month for the 1.5mg dose vs $199/month for a new patient's first 2 pen fills and $349/month for the pen after that. For patients who prefer oral medication or want the lowest cost path, the pill is now a viable option. Most commercial insurance plans that cover Wegovy cover both formulations.

What if my insurance denies Wegovy coverage?

First, appeal. Your prescriber can submit a prior authorization request with clinical justification (BMI, comorbidities such as heart disease or sleep apnea, prior weight-loss attempts). Most commercial plans require prior authorization before covering Wegovy. If the appeal is denied, your options are: use NovoCare Pharmacy Self-Pay (pill $149-$299/month, pen $199-$349/month), or discuss alternative medications your plan does cover (like Zepbound if that's on your formulary, or a lower-cost non-GLP-1 option like phentermine or Contrave).

Does Medicaid cover Wegovy for weight loss?

Coverage varies significantly by state, and many state Medicaid programs exclude Wegovy for weight management. We previously published a count of states covering GLP-1s for obesity; that count came from third-party reporting we did not re-verify on 2026-08-18, so it has been removed rather than repeated. Medicaid coverage for cardiovascular risk reduction (the second FDA-approved indication) is more common. Check with your state's Medicaid office directly for the most current coverage details.

Novo Nordisk expanded the Wegovy pill self-pay program. Re-verified on 2026-09-01 against the NovoCare Pharmacy Wegovy page and the NovoCare Wegovy savings-offer page: 1.5 mg is $149/month, 4 mg is $199/month, and 9 mg and 25 mg are $299/month. NovoCare now states these tiers with no end date on any of them.

Medicare GLP-1 Bridge launched - Wegovy IS covered
CMS launched the Medicare GLP-1 Bridge on July 1, 2026; it runs through December 31, 2027 and gives eligible Medicare Part D enrollees certain GLP-1 medicines for $50 per month. Wegovy (injection or tablet) is on the covered list, alongside Foundayo (tablet) and the Zepbound KwikPen only. Eligibility is narrow: you must have Part D, you must NOT already be able to get a GLP-1 through your own plan, you must NOT have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease, and you must be 18+ with a BMI of 35+, or 30+ with certain heart failure, hard-to-control high blood pressure or chronic kidney disease stage 3a+, or 27+ with prediabetes or a prior heart attack, stroke or blocked arteries. The $50 does not count toward the Part D deductible or the yearly out-of-pocket limit.

Patent expiration (generic semaglutide eligibility)
The earlier note in this record, which gave a much nearer expiry year, was wrong. The primary US semaglutide compound patent, with its patent term extension, runs to 2031, and Novo Nordisk's own FY2025 Form 20-F puts US compound patent expiry for the semaglutide brands at 2032, with method-of-use patents running later still. No generic semaglutide is approved or marketed in the US, and none is imminent.
Status: Anticipated. Monitor FDA approvals annually.
