Headline findings

The A-grade gap

Zero U.S. GLP-1 clinics (0%) earn an A-grade in 2026. The highest-scoring clinic in America reaches 74/100 (a B grade). Only 55.3% earn at least a B-, and 1.6% sit at D-grade.

The differentiator

Patient review volume is the only factor that meaningfully separates top from bottom clinics. Top-quartile clinics average 95.7/100 on review count (log-scaled) versus 45.6/100 in the bottom quartile (a 50.1-point gap). Other factors show single-digit differences or none at all.

Multi-channel lift

Clinics offering both telehealth and in-person care average 66.6/100 (791 clinics) versus 64.2/100 for in-person-only clinics (6,302 clinics), a 2.4-point lift.

Geographic spread

Nevada leads with an average trust score of 66.2/100 (67 clinics). Alaska ranks lowest at 58.8/100. The spread between top and bottom state averages is 7.4 points.

Findings reflect publishable trust scores (7,093 of 9,699 active clinics). Clinics with fewer than five populated input factors are excluded. See the methodology section for the full formula and exclusions.

What separates a top-quartile clinic from a bottom-quartile one?

For each of the nine input factors, the average normalized score (0–100) in the top quartile vs the bottom quartile, with the delta. Factors are weighted 3% to 35% in the composite score.

Verification, ABOM certification, claimed listings, and BBB accreditation show 0/0 in both quartiles because the underlying flags are extremely rare across the population. Fewer than 1% of GLP-1 clinics carry any of these credentials. The differentiation between strong and weak clinics is overwhelmingly driven by patient review volume.

Methodology

Source population. Clinics are sourced from the CMS National Provider Identifier (NPI) Registry and Google Business profiles, then verified to have an active website mentioning weight loss or GLP-1 services. The directory contained 9,699 active clinics at the time of generation.

Trust score formula (v1.1.0). Each clinic receives a 0–100 composite trust score from nine weighted factors: Google rating (35%), review count log-scaled (25%), photo gallery (10%), GLP-1 medications offered (8%), business hours populated (7%), verification status (5%), ABOM-certified provider (4%), claimed listing (3%), and BBB accreditation (3%). Each input is normalized to a 0–100 scale before being multiplied by its weight and summed. The composite is rounded to an integer and mapped to a letter grade. Inclusion criteria.A clinic appears in this report only if it has at least five of nine input factors populated (data coverage ≥ 5). Clinics with sparse data are flagged as "incomplete" and excluded from rankings. State rollups exclude states with fewer than ten scoreable clinics. The national top-50 ranking breaks ties first by data coverage (more populated inputs preferred), then by Google review count.

Quartile definition. The factor analysis sorts all scored clinics by composite score descending. The top quartile is the upper 25% (scores in the top 25th percentile); the bottom quartile is the lower 25%. Per-factor averages within each quartile use the normalized 0–100 input value, not the weighted contribution.

What is excluded. Brand-name retail pricing, manufacturer self-pay programs, insurance coverage status, and patient outcome data are not part of this report. Quality here is measured as a consumer-facing trust score derived from publicly observable signals. It is not a clinical outcomes ranking.

Limitations. Self-reported clinic attributes (telehealth flag, medications offered, BBB accreditation) may be stale at the moment of computation. Google ratings and review counts reflect a snapshot at the most recent crawl. The model does not weight provider credentials beyond ABOM certification or correct for review-volume bias toward older, urban clinics. Scores are recomputed weekly.

Reproducibility. The full dataset (state rollups, top-50, channel stats, grade distribution, factor analysis) is published as a CC BY 4.0-licensed JSON file linked below. The trust-score formula version is included in every record for downstream verification.
