The number-one question people ask about GLP-1 weight loss drugs is not whether they work. The clinical data settled that years ago. The question is whether anyone can actually afford them.

For most of the past three years, the answer was discouraging. Wegovy listed at $1,349 a month. Zepbound at $1,060. If your insurance said no, you were stuck choosing between a mortgage-sized drug bill and going without. That math has changed dramatically in 2026.

The Cheapest FDA-Approved GLP-1 Options Right Now

Here is what you can actually pay today, no insurance required:

Oral Wegovy pill (semaglutide) launched in early 2026 as the cheapest FDA-approved entry point ever for a GLP-1 weight loss medication:

  • $149/month for starting doses (1.5 mg and 4 mg) through NovoCare Pharmacy or GoodRx
  • $299/month for maintenance doses (9 mg and 25 mg)
  • Available at 70,000+ pharmacies nationwide

Wegovy injectable subscription plans launched March 31, 2026. Novo Nordisk is now selling the pen directly through partner telehealth platforms:

  • 12-month plan: $249/month (saves up to $1,200 per year vs. monthly pricing)
  • 3-month plan: $329/month
  • Available through Ro, WeightWatchers, and LifeMD, with more providers expected soon

Zepbound (tirzepatide) self-pay vials through LillyDirect:

  • $299/month for the 2.5 mg starting dose
  • $399/month for 5 mg
  • $449/month for maintenance doses (7.5 mg through 15 mg)

Telehealth Providers Under $300/Month

Online GLP-1 providers have become the most accessible path to treatment for people paying out of pocket. Here is what the major platforms charge:

Provider Monthly Cost What's Included
Hims (compounded oral) $49 first month, then $99/mo 5-month plan, medication + consults
Hims (compounded injectable) $175/mo Flat pricing regardless of dose
GobyMeds $119/mo 3-month starter bundle
Ro Body Program $145/mo membership + $149+/mo medication Varies by dose
Found $99/mo membership + $189+/mo medication Separate billing
MyStart Health $149/mo 3-month plan with price lock
Henry Meds $297/mo All-inclusive, no membership fee

Important note on compounded options: the FDA removed semaglutide from its drug shortage list in February 2025, which limits who can legally receive compounded versions. Compounders can still serve patients with documented individual medical needs (different dose requirements, ingredient allergies), but the broad availability that existed in 2024 has tightened. Ask your provider whether their compounded product meets current FDA guidance.

Insurance and Savings Cards

If you have commercial (employer or marketplace) insurance, manufacturer savings cards can drop your copay to as low as $25 per month. Here is how they work:

Wegovy savings card: Reduces copay to $0 to $25 for commercially insured patients whose plan covers Wegovy. Maximum savings of $100 per 1-month fill. Available through NovoCare.

Zepbound savings card: Reduces copay to $25 for commercially insured patients. Annual cap of $1,300 in savings. Available through Zepbound.com.

These cards do not work with Medicare, Medicaid, or other government insurance. If your commercial plan denies coverage, consider appealing. Prior authorization denials get overturned frequently with a letter of medical necessity from your doctor. Our guide on employer insurance and GLP-1 coverage walks through that process.

Medicare and Medicaid Coverage in 2026

Medicare GLP-1 Bridge Program launches July 1, 2026. Qualifying Medicare beneficiaries will pay a $50 monthly copay for Wegovy or Zepbound. The program runs through December 31, 2026, with the broader CMS BALANCE model expected to extend and expand coverage into 2027.

Medicaid coverage varies wildly by state. As of early 2026, only about 13 state Medicaid programs cover GLP-1s for obesity. Some states are expanding access while others are pulling back:

States with some coverage: Massachusetts, Minnesota, Mississippi, New York, North Carolina, Wisconsin, Texas, Washington, Arizona.

States that dropped or restricted coverage in 2026: California, Delaware, Pennsylvania, New Hampshire, South Carolina. Michigan restricted coverage to morbid obesity only.

If you are on Medicaid, check with your state program directly. Coverage rules change frequently and the CMS BALANCE model may expand access in your state starting May 2026.

How to Find the Lowest Price in Your City

GLP-1 pricing varies significantly between providers in the same metro area. A clinic in Denver might charge $250 a month while one across town charges $600 for the same medication. The difference usually comes down to whether they prescribe compounded or brand-name formulations, whether they bundle services, and their overhead costs.

Here is how to comparison shop:

  1. Start with telehealth. Online providers almost always beat local clinic pricing because their overhead is lower. If you are comfortable with virtual visits and self-administered injections, this is your cheapest path.

  2. Check our directory for local options. Browse GLP-1 clinics in your city to compare pricing, insurance acceptance, and patient reviews. Popular starting points:

  3. Ask about total cost, not just drug cost. Some clinics advertise low medication prices but charge separately for consultations, lab work, and follow-ups. Get the all-in monthly number before committing.

  4. Use pharmacy discount tools. Even with a prescription from a local clinic, you can fill it wherever prices are lowest. Costco and Walmart have negotiated special GLP-1 pricing for their customers.

  5. Consider the oral pill. If needles are a barrier, oral Wegovy at $149 per month is now a legitimate option and eliminates injection supplies and training from your cost equation.

The Bottom Line on Affordability

Twelve months ago, paying less than $500 per month for a GLP-1 required either good insurance or a compounding pharmacy. Today, FDA-approved options start at $149, subscriptions bring injectables down to $249, and Medicare coverage is months away.

The pricing landscape is still moving. Novo Nordisk announced list price cuts of roughly 50% for Wegovy taking effect January 1, 2027. More telehealth providers are entering the market every quarter, pushing prices lower through competition.

If cost has been the thing stopping you from exploring GLP-1 treatment, run the numbers again. The math looks different now.
